Is an on-premise AI system HIPAA-compliant?

HIPAA compliance is a property of your whole environment, not of any product — but keeping PHI on hardware you already control removes the hardest part of the analysis. There is no business associate agreement to negotiate with a model vendor, because no PHI leaves your network. We deploy with role-based access and query audit logging so the controls are documented.

Can we use ChatGPT for patient records instead?

Not without a signed BAA covering that specific product tier, and not with the consumer version under any circumstances. Even where a BAA exists, you are extending your compliance perimeter to a vendor. That may be an acceptable trade — we will tell you honestly when it is — but it is a decision to make deliberately, not by staff habit.

What can a clinic actually use this for?

The workflows that pay for themselves fastest are intake summarization, answering questions across visit notes and charts, policy and coding lookups for staff, and drafting routine patient communications for clinician review. Do we need to buy hardware?
